Educator Request Form

If you are a WV public-school educator and would like to submit a request for a military service member to speak to your school or classroom, please provide the following consent and complete the request form. 

Military partners require the submission of this speaker series request form at least 35 days (5 weeks) in advance of the desired date on which activities are scheduled to occur.

I agree to provide my information to partners identified in the Common Ground Compact (the West Virginia Department of Education, West Virginia school systems and military organizations) for the purpose of being contacted about guest speaking opportunities and scheduling a school visit. 

I understand that these requests are used for the sole purpose of establishing initial communications between schools or school systems and military organizations. 

I understand that upon receiving contact from military organizations, it is the responsibility of school staff to establish the terms, conditions, and supervision of military guest speaker activities and to monitor those activities to ensure that they are conducted in accordance with all applicable laws, policies and established protocols. 

I understand that making this request does not obligate the West Virginia Department of Education or partnering military organizations to meet the request, as their ability to meet needs of the school may be impacted by a variety of factors including, but not limited to, the nature of the request, ongoing missions/training and soldier availability.
