Clinical Teacher of Record Permit
Summary of Requirements
Form 3 allows individuals to teach in an area of critical need and shortage while finishing their state-approved teacher preparation program as part of a WVBE approved agreement between an institution of higher education and a WV public school district.
Full Requirements
- Validity Period: 1 year
- General Requirements: US Citizen or hold eligible documentation (see WVBE Policy 5202); good character; age 18; physically, mentally, and emotionally qualified.
- Grade Point Average: Minimum overall cumulative GPA of 3.0 is required in the area of specialization
- Coursework/Training: Meets coursework/training requirements of teacher in residency agreement between the IHE and County.
- Testing: Individuals must pass all WVBE required exams for basic skills and content proficiency. Applicants must pass WVBE required Principals of Learning and Teaching exam or educational teacher performance assessment as required by the IHE upon program completion to apply for a professional certificate.
- Employment Required: Offer of employment as a teacher-in residence, based upon qualifying employment.
- Official Recommendation: Superintendent in the county in which the applicant will serve as the Teacher-In Residence AND the Designated College Official
Application Instructions
Form 3 must be completed online. If this is your first time using the WVDE Certification Portal, you will need to register. Follow the instructions for first-time applicants.
- Log in to the portal and select the "Renew" button for the certificate you would like to renew.
- Select the option to renew your certificate.
- Fill in the required information. Depending on the option you choose, you may need to include information on your work history. Please note that you may need to submit documentation verifying your experience. At this time, you will submit the application.
- If you are employed in a WV county school, your application will go to your county for approval. Following county approval, you will receive an email for payment. Log back in to the portal to pay the application fee.
- If you are not employed, the "pay now" button will appear on your dashboard, and you will be able to pay immediately.
- Following a review and approval of your application by the Office of Certification, you will receive an email with instructions to download your certificate. You may follow the status of your application on the Certification Portal or by checking the Application Status.
- If you are a first-time applicant for licensure (have never held a license/certificate issued by the WVDE), you will receive an email with a service code to make your background check appointment once the application is received by our office.
