Certification Applications

Use this form if you are employed by us, but do not meet the requirements for a professional certificate, and are enrolled in a state-approved educator preparation program.

This permit is for part-time employees in a specific endorsement in the K-12 setting who works full-time in a college/university instructional setting in the requested endorsement to teach that course to K-12 students upon recommendation of the county superintendent.

A county board of education may issue a Contractor/Volunteer or other School Personnel Permit to you if you

Advanced salary/advanced degree recognition is limited to Professional Teaching, Student Support, or Administrative Certification or equivalents.

If you attended an in-state institution, this certificate allows you to work in WV public schools as a Principal, Supervisor, or Superintendent.

If you attended an out-of-state institution, this certificate allows you to work in WV public schools as a Principal, Supervisor, or Superintendent.

Convert a Provisional Administrative Certificate to a Professional Administrative Certificate.

If you have a valid out-of-state professional administrative certificate, this certificate allows you to work in WV public schools as a Principal, Supervisor, or Superintendent.

Use this form if you are employed by WVDE, but do not meet the requirements for a professional certificate, and are enrolled in a state-approved alternative educator preparation program.

Use this form in-lieu -of the Form 1 for Autism when an individual currently holds a Special Education endorsement and is taking the six hours of coursework from an accredited IHE that offers the six hours of coursework but does not have a full Autism endorsement program.
