Option One: You are on a continuing contract, hold a Professional Certificate that must be renewed, AND hold a salary classification of MA +15 or less; OR
Option Two: You have completed coursework in a county-verified shortage area.
Please Note: There is a 15 semester hours reimbursement lifetime maximum.
Form 36 applications are accepted July 1 through June 15 of each fiscal year. They are processed on a first-come, first-served basis until the designated funds for that fiscal year are expended. All coursework must be completed within the same fiscal year in which WVDE receives your request for reimbursement.
Applicant Requirements
General Requirements:
US Citizen or hold eligible documentation (see WVBE Policy 5202)
Employed by WV public schools
Hold a professional teaching, student support, or administrative certificate or first class permit
Minimum 3.0 GPA is required for coursework
Be seeking an additional endorsement in a shortage area, if applying under Option Two

Download the form and fill out the applicant information, disclosure of background information, and the details of the courses you are claiming for reimbursement.
Please note the required information. You must include:
A receipt verifying payment
A transcript or grade report
This signature of your County Superintendent verifying that you are eligible.
Ensure both you and your County Superintendent have signed and dates both pages of the form.
