Form 4B takes the place of a superintendent recommendation. It must be submitted before any other application can be processed.

Application Instructions

If this is your first time applying for a certification with WVDE, you will need to complete Form 4B as part of your initial application. Follow the instructions for first-time applicants.

1. Download the application form.
2. Fill in the applicant information at the top. 
3. Request an individual not related to, nor living in your household, to complete ONE column in the second section of the form:
   1. If you have been employed in an educational position in the last year, the Recommendation Based Upon Educational Experience column will need to be signed by a former supervisor.
   2. If you have NOT been employed in an educational position in the last year,  \the Recommendation Based Upon Non-Educational Experience column will need to be signed by someone who has known you for at least a year. They cannot be a relative or someone you live with.
4. Submit the form and all supporting documentation to the Office of Certification. Please mail to the following address:
