Use this form if you meet all requirements for completion of an approved West Virginia Board of Education county-based Alternative Certification Program, or you hold a valid West Virginia Statement of Eligibility for employment in a West Virginia public school.
If you are converting from an Alternative Teaching Certificate to a Provisional Teaching Certificate, you must have continued employment in a West Virginia public school. If converting from a Statement of Eligibility to a Provisional Teaching Certificate, you must obtain employment in a West Virginia public school to qualify.
General Requirements
To apply, you must:
Be a U.S. citizen or hold eligible documentation (see WVBE Policy 5202).
Be at least 18 years old.
Be physically, mentally, and emotionally qualified.
Have recommendation from the county Superintendent.
Hold a valid Statement of Eligibility or Alternative Teaching Certificate.
If converting from an Alternative Teaching Certificate: Successful completion of all requirements of an approved West Virginia Board of Education county-based Alternative Certification Program, including the appropriate recommendation on application Form 25C.
NOTE: There are no test requirements if you are converting from a Statement of Eligibility.

Pay the processing fee online before submitting your form.
Submit your application to your employer. Your application must be uploaded by your employer’s human resources office.
